Enrolling a new student? Make sure to bring these documents:
Student’s certified birth certificate,
Immunization records,
Proof of residence within the school district boundaries, or bring your district transfer approval.
The last report card or withdrawal form from the prior school district (if available), and
The student’s Social Security Card (if available).

CRHS Sports Medicine will be offering sports/fine arts physicals for $20 on May 12th from 5:30 - 8:30pm at Cedar Ridge High School Main Gym. Enter through the bus lane doors. Physical Examination Forms will be passed out to student athletes the week prior - the Medical History side of the form must be completely filled out & signed by parent and student before the student arrives.
